Once the HSA1 form is filled in and signed then the abortion is approved. It can not go ahead until then - i.e. it would be illegal.

Assume for the point of arguing that the scenario did happen. Nobody can find the paperwork and nobody can remember completing it, but the two doctors definitely discussed it and agreed that the abortion met the criteria.
It later transpires they were called away to an emergency and forgot to sign the certificate of opinion.

Of course they do, doctors can allow it to proceed, or not, by certification, i.e. approval.As after all, nobody needs the approval of a doctor to proceed with an abortion.
