Stanley knife. Cut into both edges of grout line separately, then it hacks out easily. I also use an electrical test screwdriver to scrape it out. You will also need elbow grease. How much grout you remove depends on whether you are removing it to replace a tile or just re-grout dirty grout. I use a folded edge of sandpaper for the latter.
