I had all my drafty single-glazed sash windows replaced with new double glazed windows /frames a year ago. Since then, the one remaining single-glazed thing (a door) mists up in the winter. I don't want to go through the hell of removing the frame, so I am planning to just replace the door...

Can someone get me started, I've done a lot of googling and am swamped with inapplicable information.
I want to :
-replace the single-glazed glass in a single (wooden) balcony door with a sealed double-glazed unit.
-replace the skylight glass in the frame above the door...
