It's leaking anyway, so you have nothing to lose by removing the capping. Or get some rubber flashing and glue it to the wall/lead flashing and overlap the capping.

Which way is it supposed drain off. There doesn't seem to be a path for the water to drain off the roof. Looks like a lot of garages together.

I would have thought that having a membrane below the tiles/lead valley would provide an additional safety factor, just in case any water leaks in.

Yes, a rubber coupling should do the trick. You need a soft material to make the seal. Or better still, get a longer section of pipe so that there are no joints.

How did you connect the bathroom ceiling vent to the ducting?

The 2nd fan seems to be to provide a boost. But when the 2nd isn't switched on, it will impede the free flow of the air.

The filling/top-up loop could be located at the same level as the highest radiator in the house. Just saying.
