    You almost certainly have terracotta vents. The mould is due to cold bridging through the vent walls. Tricky to eliminate entirely but may indicate that you room has a higher level of water vapour than normal.

    The diagram is wrong. The lintel is a tie, to comply with regulations, the next course of ties should be 450 above the lintel. I wouldn't put a course of ties on the cavity tray.
