I recently turned a BG job down for all the reasons that are being stated here. Mainly a company that will drop you in at the deep end and if you don't perform or meet the targets will spit you out again ...

My guess would be that it needs new thermisters. Probably the one on the primary flow. Maybe scaled up if the heating has not been on for a while. I'd replace both of them at the same time.
You need a RGI as the combustion chamber needs opening up to get to the heating thermister.
Condensate pipe has been fitted incorrectly !! Also manufacturers usually state that max length of condensate pipe is 3m.
By the way to me that 'overflow' looks like a solvent "t" to my eyes.
A shame really as airing cupboards are usually near to bathrooms and with a little creative...
Maybe as you said your filling loop is 'letting by' Remove it and check.
Maybe not the expansion vessel as you say the pressure rise happens when the boiler is at rest.
Could also be a pinhole in the secondary plate heat exchanger allowing cold mains water to mix with the heating side.

I'm not too familiar with these old boilers but have changed the diverter.
It could be the syncron motor so i would swop this over first. You can test then with a meter or a flying lead but you need to know your way around electrics.

This is a simple pumped gravity system really. Needs upgrading at some point but i understand how money is tight at the moment.
Boiler heats water (boiler thermostat controls the temp) and pumps it around the heating side through your rads until it "flows" into the room with your hot water...
A common problenm with Halstead combi's. If its not been used for a while the gas valve has a habit of sticking.
Are you sure the venturi was properly cleaned out???

You say you changed the domestic hot water diaphram
There are two diaphrams i think in this diverter valve assembly.
One in the domestic hot water flow valve at the front and the other in the primary flow valve at the rear. the pushrods may also be sticking and need cleaning.
Looks as if it needs a new pressure release valve.
However, also need to find out why its leaked in the first place. May be just grit/muck on the prv seal.
When whoever changes the prv, get them to also check the expansion vessel.
