I've just done mine today and I was previously getting a maximum of 16lpm on an ancient run of 1/2" galvanized. I now get 27lpm from the garden tap (through 15mm internal pipe). Probably not similar setups to allow any meaningful comparison, but shows what is possible.

A Philmac tee (part 98933300) with a 25 to 15mm copper connection kit (part 99603200) will do the trick.

Don't panic, it's more than likely caused by a build up of kitchen waste in the pot at the bottom of the gulley. It's a horrible smelly job, but get a pair of shoulder-length gloves and dig out whatever is in the base of the gulley (it's just a big u-bend). You should find that the water flows...

First of all, sorry it hasn't worked out for you. A washing machine draining a drum full of water shouldn't cause an overflow - as I mentioned, I have that setup pictured and I have no problems draining both appliances at the same time. I'd suggest you have a problem further downstream...

Might look like a bit of a stupid question, but I'm installing a new 32mm MDPE supply pipe and I'm not sure what bore insulation to choose. Having looked at the spec sheet, it seems that 28mm bore is good for up to 31.5mm pipe. The 35mm bore insulation is good for 35mm onwards (obviously)...
