    Wall tie replacement in cellar

If it is taking a lot of weight from the ground on the outside why is it even a cavity wall? I'm no expert but it doesn't sound like a cavity wall would take the pressure and the wall ties would just crumple under the weight, not support anything. I would have thought a tanked solid wall would...

    Building a hearth

You can use anything - obviously fire proof if using the fire. Slabs, slate, granite, bricks, marble etc. etc. etc. The floor will act as a constructional hearth so just put the decorative on top.

    Chimney breast problems

That's women for you. If you are going to the trouble of disturbing brickwork and re-positioning lintels why not support at ceiling level and take the entire breasts down to floor level? Or take the stacks and breasts out completely while you are at the 'messy' stage.

Not structural - just take it to bits (rip it apart). Yes - no problems there. Obviously you will need to board in the underside of the stairs.

    Lowering a ceiling

Don't bother with joist hangers, just screw a piece of 50 x 100 to the walls and secure joists to that; like a horizontal stud wall instead of a vertical one

Most likely as you say, a tiny amount of settlement and contraction. A good quality exterior filler should take care of it.
