    As mentioned by previous posts it will be 100% the diaphragm, common issue with siamp ballvalve. They can be replaced and quite an easy job for a diy fix.

    Please tell me a genuine plumber has not installed that? If he was I would suggest you try and get your money back, or get him back to do it properly. (the hose onto the existing cold isolation valve is unbelievable) I hope you get it sorted out in the end.

    It could have an internal overflow pipe, which lets the water overflow into the pan.

    It will sods law that if you tile up to it the toilet will then at some point break and need replacing. I would personally get a cheap toilet pan and replace it after floor tiled. Won't be expensive, just more time needed.

    As above, if its an old outside toilet the pipe could be off next doors supply. Had the same problem a few times.

    Then put a MASSIVE! boxing round it and tile it, so it's really awkward if you need to repair it in the future.....that's what I normally come across anyway.
