Depends if the customer has a preference for the mixer height to be set at, if not then I go with anywhere between 1000-1200mm, riser type - rigid, rigid telescopic, separate rail etc - and then ceiling height dependent.
Standing height, so from the shower tray.
Stand in the shower and gauge where you would be comfortable with the controls. I fit usually just above waist height unless the customer specifies.
