1:1:3 of lime cement sand will do. If you have to flaunch the chimney terminals then bulk ip the mix with a little aggregate.
Your biggest difficulty as a DIY'er is gaining safe access to the stack, raking out - with a grinder - and removing any debris.
By the time re-pointing is req. then the flashings will req. attention too.
