for my concservatory, is the following correct

hardcore 4 in
sand 2in
insulation 4in
concrete 4in
self levelling compund ? in

so, how much do i allow for top screed, and where is the best place for the dpm

What floor covering do you intend to use? The DPM usually goes on top of the sand and lapped into the DPC (not cut flush with the slab as done in the 70's). You could finish the concrete off well and do away with the screed.
 
tiles, so good finish i suppose would do away with self levelling

Go for a really good finish to make it easier for tiling. You will only need self levelling compound if your concrete is like mine after 6 pints of scrumpy.
