In a combined system, if there is a drain with one toilet connected to the sewer and this has an inspection chamber, does another another drain with a second toilet, connected to the sewer a few feet away, have to have it's own inspection chamber? In other words, if you have two connections to the sewer can you simply have one inspection chamber per house?

Often the case with older systems where laterals were connected without a manhole on the junction. Basic rule of drainage is all pipework must be accessible for rodding should the need arise.
If you're planning on adding a connection then it may be possible to incorporate a rodding point rather than an manhole, but this work is notifiable under building regs so Building Inspector will say what he wants. Regardless of that you dont want a section of sewer thats inaccessible should worst happen.....

If going to the trouble of exposing drain & cutting in a branch for new WC you may as well fit an inspection chamber , or dependant on depth of channel in existing manhole (person-hole 
) you could fit a tumbler.
