floor tiles over wooden floor

I am thinking of fitting ceramic floor tiles in the kitchen on top of a wooden ( uneven )floor. Can anyone help with advice on preparation and what type of adhesive to use. I am also thinking of continuing the tiles into the back hall, which has a concrete floor. Will this cause any complications? I am a fairly competent DIYer. Although I am always looking for shortcuts. Any help or advice will be appreciated

If the floor's uneven don't mess around. Pull it up and replace with ply (£18 for 8x4 sheet).
 
Should have read the sticky before posting. I only have one question now I have covered the floorboards with T&G chipboard, for a previous attenpt at tiling. Willthis be OK to tile onto , or will I be better to use WBP sheets
