Hot water/ Boiler problem

My shower and upstairs taps are not producing hot water as they should. The downstairs tap next to the boiler gets hot when opened fully, but I also think that the furthest radiators from the boiler are also not getting hot enough.
The shower is fitted properly with a pressure regulator and was working fine until recently but is steadily getting worse.
Please help, getting desperate.

Boiler make/model, size and layout of house, number and sizes of rads, hot water outlets and their flow rates, make and model of shower units etc.
