The internal switch on the hot water valve actuator could be faulty ie opening valve but not telling boiler to run until heating valve opens . Will heating run on its own?? You should get 240v on both grey and orange wires from the actuator when valve is open if only 240v on one when selecting hot water only, the switching is faulty. Change actuator or complete valve if older.
has this just been installed ?
