laying concrete over existing base.

I've got an existing concrete base (approx 5" thick above ground), which at some time in the distant past has sunk towards one end by about 3". I want to erect a heavy duty wooden shed, so need to level up the base.
So I need to know
1. how do I prepare the existing surface?
2. What sort of thickness should I apply (bearing in mind the 3" level difference?
3. What sand/mixture should I use?

I realise that it would be best to completely relay the whole Base, but it is a large area, and I'm trying to avoid a lot of work if possible....
