We moved into an old terrace house. We share the sewer with our neighbour. There is currently a conservatory at the rear that wraps around the kitchen. In the conservatory is a manhole but it covered over with laminated floor so it's exact location is not known.

We plan to convert the ground floor and have the kitchen expand into the footprint of the conservatory. In the image, I've put what I think is the position of the manhole and layout of the sewer pipes (yellow) from my neighbours to the sewer in the street. There is also a gully and two downpipes (blue). The artictect is appying for a build-over licence for us. I'm told the manhole cannot be in the footprint of the house.

My question is what needs to happen to ensure that the sewer is maintainable. I guess the manhole will be moved to the rear, in the garden, but that seems like it will be a long way from where the sewer pipes run. I don't want to get caught out in a few year and have to have the floor ripped up to fix a leaky sewer.

Also what sort of trade specialising in this work? Can I entrust this work to the contractor that is doing the extension? They are ok builders but the are quite general builders.

The water authority will comment as part of the build over agreement, it belongs to them so they'll probably specify what they want.

Yes, although, again, the WA may have a say on that as well.

Shared Sewer so will be the Water Companies asset. They will dictate what they will allow you to do and how it is to be done. If your Builder is happy to do the work, then that should be ok, but the Water Company will check it has been done to their specification.
