Plastic toilet bolts

If you need to take a cistern off to replace parts on a cistern with plastic bolts like this can you replace with metal ones that have an extra bolt on them to tighten between the cistern.

Yes you can. But why would you want to ?
Because, as you should know, those plastic nuts and bolts are rubbish. When they work loose, which they will, extra tightening to stop the drip will result in the plastic threads being stripped. Yes you can use the metal ones and I always add a locknut if one isn't supplied. if there wasn't an issue with plastic nuts and bolts manufacturers wouldn't be making replacement steel ones.
 
Never had a problem with plastic bolts. The metal ones however often rust up and are a pain in the you know what to remove on certain cisterns. Why do you think metal ones even need to be replaced !

I don't think they're making replacements specially. They look like standard bolts, with one of the available bold head options, dome head most likely.

A different situation, but years ago I had a problem with a toilet seat, steel bolts which kept working loose. I tightened them till I was worried about cracking the porcelain, but no better. I eventually replaced the bolts with nylon, and no further problem. No doubt because of it being slightly elastic.
 
IMO the only nonstandard parts on those bolts are the rubber/neoprene drilled cones and they are probably swiftly obtained if you know where to look. Full marks to the guy who used his noddle and put the completed set together to make a few bob
