This stop tap looks like it has had a poor repair in it in the past. Could I change just the head of this stop tap? If swap the full valve the distance the olives on the pipe maybe different.

It's 25mm to 15mm I am hoping to just change the valve head not the whole body. Is this usually doable?

Sometimes, but not always if the threads on the head are different. Is there any markings on the valve body. That nut on the 15mm end looks cockeyed in the photo.

If you only want to change it because it's leaking, maybe tightening the gland nut will stop it.

Yes, but if you can’t swap the innards, then it should be easier in the long run.
