I remember reading a book about commentator gaffs. I think the title was, "The bowlers holding, the batsmans Willey." Obviously referring to the bowler being Michael Holding and the batsman was Geoffrey Willey. Here is the audio version with a couple of others.

I happened to catch part of the live broadcast of William Shatner's recent trip to space, it was absolutely hilarious, so so awkward, there's a shell shocked Shatner practically balling his eyes out trying to form his words and be all profound and that **** Bezos couldn't have been more disinterested, more interested in spraying champagne with his screaming groupies!
