Worcester 28 CDi RSF - Hot Water Problems

I just moved into a property with a Worcester 28 CDi RSF boiler. The problem is with the hot water. When running a hot shower, the hot water runs for a few seconds and then runs ice cold for nearly a minute.

Any ideas of what may be wrong?
