Replace Concrete Floor

I recently had a survey completed on our house and it has said that the floor in our middle room is uneven and in need of replacement as it is old quarry tiles.

Help - I have no idea what to do.

Its a mid terraced property about 100 years old.

Can anyone help?
 
Dig out about 450mm of material and lay a full spec' floor.

100mm compacted mot.

25-50mm sand blinding.

1200g dpm.

125mm jablite (or 80mm thermaflor)

100mm concrete slab (or trowel finish, omitting the screed).

75mm screed.

If I had a builder do it do you know roughly how much it would cost. The room is about 10ft x 8ft.
