That sounds very familiar, cheers geez.
I have heard people say that undercoat would do it, but this is a very nasty stain.

It has improved a bit, but keeps coming back.

Stain will ALWAYS find its way through a water-based paint , it cant however bleed through solvent based... Another way to cover it is with car spray paint - an aerosol can of white primer 
Yep, I knew that, which is why it got a thick coat of pva over the plaster, and another thick coat on top of the first white coat.
Have had pretty good results doing that when making good in the past, but no joy here.

I use cheap Berger's white oil based undercoat on stains all the time Ben, whether they're light stains or real heavy ones,, water marks,, nicotine etc. I put it on with a small roller. Smells a bit, (don't we all 
) but it works all the time, and does the exactly the same job as expensive stain blockers,,,, but at nowhere near the cost,, around £8 to £9 ish for 2.5 ltrs.
