Slow draining toilet + what’s this?

It may do, but you may also want to go to next empty manhole, depending on how far, to clear.

It’s the only manhole on this side of the house
There in another one on the back (opposite side of the hose) that I haven’t checked
The house is a mid terrace

Not sure how the systems works
 
Contacted the water company this morning and they fixed it at lunch time!

My front door would be at the top of this picture, is this a shared manhole as it was unclogged for free?

That's what I was going to suggest as you said a mid terraced

Impossible to tell from the pic, would need a lot more info, and/or be on site to tell, but as the Water Co have taken responsibility, and your Mid Terrace, it is a fair bet the drain is shared.

As Andy has said above, that still looks blocked. There shouldn't be any sewage hanging about in the channel.
