When cutting a kitchen worktop to fit between two wall in a utility. Are measuring the gap wall to wall. How many mm would be advisable to take off to make sure the unit fits first time and does it need some expansion room?

No expansion room required.
Use something to determine if the back and side wall is square as you can cut worktop tight but and angle will foul it.
2mm gap I'd say would be tight.
You can have 4mm so 2mm either end as tiles or silicone will cover..

I could use a set square?

I could use a set square
You could if it's big enough. Walls tend to wiggle a bit, corners are not square and straight bits are not straight. I usually put a large sheet of plywood, chipboard or mdf along the wall and measure the worktop dimensions and angles from that.

You can use anything at a square.
8x4 sheet of hardboard used to be £5 and I cut corner of to use as a big square as an example

This is always a challenge. The chances of the two end walls being perfectly square is close to zero, if only by a few mm. That few mm can be a PITA if you are trying to fit a heavy worktop. I wouldn't bother trying to measure angles. with a wall to wall WT there is no jiggle room. The best way is to template it in something really light first - correx, thin MDF, ply, or even cardboard then cut the WT to the same size. You will still need a few mm clearance, particularly if you are trying to put one end in and then drop the other end in.
