you can do a quick response system - pipes in 30-40mm of screed at 100 centres. mine's floorboards on joists with the screed on insulation inbetween the joists.
you would have to do insulation between battens fixed to the slab (you need about 100mm to minimise heatloss downwards) screed to top...

edit - i thought you might want holes for pipes, but maybe you want recesses (ie not all the way through) in which case these won't be suitable.
manchester building control approval is £69 inc VAT for any number of replacement windows.
you will have to be sure that the windows you get conform to all parts of the regs. the building control officers can be surprisingly helpful on advising you what to check.

bear in mind that the hinges and locks will depend on the strength of the joints you make. i would use screws long enough to go as far as possible into the existing stiles, and thick ones(5mm min). use a clearance hole on the make-ups and the screws should squeeze up the joint tight.
i would be suspicious of them having rendered the outside after the structural work. is is normal for a few hairline cracks to appear after removing walls / installing support beams. but any crack resulting from this kind of work should be monitored over time (months/years) to see if is widening...
