Assuming the tile is ceramic then spade bit, if porcelain then you would struggle with a spade so you should try a diamond tipped drill bit designed for porcelain and natural stone tiles. Both will need to be kept cool using a cup of water unless you have a Rubi Dry Gres bit lying around.

The usual is about 5 metres for a floor of BAL Rapid set. And flexible is ideal for a 2nd floor in any case. The one guy I know who did use a non flex ended up with a blown floor 4 weeks later. But I do get the ethos behind it.
Rip it back to scred and follow the manufacturer recommendations. BAL have a technical number on all their products and I've found the guys to be very knowledgeable and professional.
1 mm bed of Adhesive? The BS I believe is 3. If a tiler used a 1 mm bed of Adhesive he'd be off site quicker than a cat out of a hot pan. Now as I say this, unless the package explicitly stated that 1 mm is acceptable then I will gladly be corrected.
Has to be powder based mate. I'd say 3 mm spacing and I would personally use BAL CTF4 or Topps own slow flex as I've heard that's quite workable from some local traders. Not sure about Mapei brand slow sets but they are also quite reputable.
Mastic? I hope your not referring to silicone as that would be borderline insane to use. Cement based adhesive is ideal for this kind of job but I'd suggest using a flexible based one such as BAL Max Flex or Mapei S2 depending on budget.
Parquet flooring isn't ideal for tiling onto either. I'd suggest either using BAL Fastflex which is suitable for straight over wood (still score it) or going the long hard route over tile backing boards.
I would use BAL Micromax Spray (not sure if this was just in topps) or FILA Grout cleaner. I buy the grout cleaner and sealer whenever I move house and inevitably lose my old stuff. Sealer means that I thw dirt doesn't get mashed into my kitchen tiles.
