im an electrician, ive been asked to put a 4" fan onto a already tiled wall, its a timberframed house.

Diamond core drill will be the easiest method (for the tile at least) or you could resort to lots of little holes around the perimeter but I’ve never done that with the tile stuck to the wall before. Do not us the hammer attachment or you will almost certainly shatter the tile & if they are porcelain (hard), your going to have a job on your hands.
