advice on converting garage

I want to convert the garage, currently there is a concrete floor and I will look to raise this up approx 6 inches to meet the existing house floor. Can anyone advise on best insulation and how to lay it? Will using an insulating form and then a concrete screed be the best option? thanks
 
When I did this, I used joist hangers, 50mm insulation between joists as recommended by building control, and t+g flooring board, adding through ventilation bricks to outside, and to the house existing subfloor space - which had no insulation as originally built.
