You can remove the existing air brick, fit your duct pipe in the hole and mortar or foam it in.
At the other end, in your new cavity, you fit an adaptor from round to rectangular, and this goes onto a telescopic 'periscope' vent, which goes up through the cavity, and lines up with your new air brick.
