Are these floor details correct?

At the moment I have a wooden floor in the sitting room but all other floors in the 3 bed semi are concrete. Under the floor there is damp and it has effected the timbers (No through vents due to others being concrete). So I am going to replace it with a concrete floor.

Can you tell me if the following is correct:

75mm Screed
100mm Concrete
100mm Rigid insulation
DPM
100mm hardcore (sub-base)
Soil

I have two layer black engineering bricks in the walls as a dpc. How high up the wall should I bring the dpm and should I insulate the sides of the concrete slab as well as under it?
