Bedroom Floor - Replacing Chipboard with Ply.

Following on from my earlier post about noisy bedroom floors, the consensus seems to be that chipboard is not a very good as a flooring product. So I plan to lift the existing chipboard flooring, scrap it and replace with plywood. So I would be grateful for help with what spec the ply should be:

Is there such a product as floor grade ply?
What thickness should it be?
Should it be T&G?
How should it be secured to the joists?
Any other special fitting tips?
Is there a recommended supplier in the Bristol area?

You can get ply flooring panels with a t&g, same size and thickness as 22mm chip. About 3 times the cost but much better quality.
Google/yellow pages for local suppliers, a timber specialist would be the first place to start.
