I have above printer that will soon be needing a new drum/ cartridge (I think). Some lines are not being printed and printer makes groaning noises.

The printer is usually quiet (no groaning noise) when the drum is replaced. At last drum change, it continued to make this strange noise.

Should I be replacing the printer or will cartridge/ drum replacement result in quiet operation once more?

How long is a piece of string? Can you run it with the drum module removed?If changing it last time did not stop the noise why do you think it will this time? Does the noise affect the operation of the printer? Can you live with the noise? I think you have answered these questions already but thats the only way you can decide whether to replace the printer or not
