cavity insulation lower level

Hi everyone. I am building a small ground floor extension. I understand that the cavity needs to be filed with weak concrete from ground up to 2 courses below DPC. My question is,
at what level does the insulation go in ? Does it sit on top of this concrete or start at DPC ?

IIRC it stops three courses below the dpc.

Fill the cavity from the top of the concrete upwards, even though some is below the dpc.
 
many thanks Tony, the reason I asked was because of concern regarding insulation getting wet and allowing moisture to bridge the gap. I suppose it would depend on the type of insulation used ?
