Concrete driveway?

Good morning :)

With regards to a concrete driveway, what is the proceedure?
This particular garden has been pre dug & shuttered leaving exposed mud, what sub bas should I use? Type 1? And should I use a plastic membrane?

Any advice greatly received!!
