Concrete Floor, Insulation & Sheet Vinyl,? DPC required?

Hey Folks

We have old house 1901 , its has concrete base and the current kitchen tiles are straight to the concrete.... very cold haha.

We want to install, Thick sheet Vinyl , What is the best way to lay this : (not taken it all up yet, so floor might no be smooth)

1) put down some foam or rubber insultation for thermal and sound. (any recommendations)
2) Plywood top of foam then thick Lino on top of plywood?

Do I need to put any vapour barrier down,? to protect vapour to floor or floor to kitchen? which ever way its flows to prevent damp, mould etc

Think I saw some of your other posts, I see they are foiled backed I presume that the vapour barrier?
 
You get different backing and none are a complete vapour barrier.

What tiles are down now ?
