damp chimney breast after fitting register plate

I would have thought you'll have a standard pot sitting on top of the chimney brickwork and then flaunched around. It's fairly common for the joint between the pot and the flaunching to crack and allow water in, which then runs down the liner and settles on top of the register plate. If you can get at it a good quality polysulphide sealant will sort it out. (polysulphide because it's semi setting and won't dry out and crack in the sun)

Should the flue liners terminate with a flat plate to cover the chimney and then have the pot cemented on top can anyone tell me please? thanks.
Usually the closure plate and clamping ring will be fitted below the oversailing course, and then the pot put on top and built in with a couple of courses and flaunched. In some countries they allow an expansion gap between the flaunching and pot and fill with a sealer.
