Damp Spots in Floor - Advice on how to replace

Hi

I am new here and just wanted some advice please, on my (fairly) recently purchased property.

I have just had the guttering completely replaced but have now just noticed that the small extension on the back of our house is suffering from some of the plyboard/plywood under the lino flooring being damp in places. I presume this is moisture rising from beneath?

I want to replace this and judging by the time the extension has been up (10years +) and the fact the previous owners seem to have bodged every problem I have come across I wanted some advice. I am assuming there is either no DPM (allowing moisture to rise up) or it has given up the ghost. I will be removing a few boards at least, this weekend to inspect

Assuming the base of the floor is concrete....

Do I lay a DPM over the concrete base and then lay specialist flooring plyboard/wood on top of that. Will I not damage the DPM by screwing into it? Obviously the walls are already constructed so how would I fully lay the DPM?

Do I need added insulation in the above scenario and if so where would it go?

Is there a better way of doing this before I lay the final flooring (laminate) ?
I have read about screeding but am unsure what its purpose is. Can it serve as a DPM if done with certain materials?

Why not research recent similar posts on here, and then remove your board(s), and then come back with your findings and pics if possible?
