Definitive guide to laying concrete floor

Hi, Can anyone give me a definitive guide to laying a new concrete floor?

The concrete floor in my house has sunk about 3 inches in the middle of the room and there is 'efflorescence' in numerous places since all carpets have been taken up. The house is circa 1965 and I would assume that any settlement (of the concrete) would have taken place by now?

Do I screed over the existing floor with self levelling screed and apply some sort of DPM or do I rip the existing floor up and start again? If the latter where do I start with regards to putting it back together? I've read that insulation is required along with DPM - is this necessary?

Can anyone advise with regards to the above? I'm starting to get a bald patch from scratching my head!! :?
