Ferroli Mondena 80E

About 3 weeks ago I had the expansion vessel replaced on a Ferroli Modena 80E. All worked fine until last night. The hot water comes out brown for around 30 seconds, it then clears but the heat cuts out. Central heating no longer works because the pressure has dropped to zero. Haven't got round to re-pressurising yet. Could the exchange have failed letting water through from the heating circuit? I've read that the 80E is a dual exchange so a failure may result in CHW contaminating DHW.

If that were the case, the boiler pressure gauge would show 3 bar pressure
