Could be a blocked plate, could be a faulty temperature sensor, could be something else. Not DIY though I'm afraid as to access the relevant parts for testing the cover needs to be removed, and this should only be done by a Gas Safe Registered engineer (who can also check at the same time that the nasty rubber gas pipe inside it hasn't split, which they tend to do, and that the burner gasket is safe because they have a habit of perishing and leaking).
Turn the flow rate down, and see if it steadies itself, that'll indicate gunked up heat exchanger. Then turn down the boiler hot water heat setting, and see what that does.
