How do I make good this Damp Proof Course (Pics included)

Hiya,

From the looks of it, this damp proof course needs help - the question is, can I do it myself?

I was thinking of injecting a chemical DPC and then repointing the cracked and spalled areas - but that's just a totally wild guess. Can someone correct and inform me?
