Careful with the use of the word "prevent" - there are certainly various degrees of reduction depending on the thickness of board used, but lacking any specifics your post might lead someone to think if they whack a 6mm jackoboard in somewhere and magic; no more condensation risk
that's the only section from the S.E. As I dont'have an Architect I am relying on 'competent builder'... One thing that puzzles me is how do specifiers deal with the fact the moisture could condense and or run down onto the steel plate over the cavity ... and just sit there....
 
Weep vents are usually inserted into the bricks on top of the lintel, to allow accumulated water to drip out. There should also be a "tray", which in reality isn't like a tray at all but a sheet of polythene carefully crafted into a funnel shape.
