kitchen sink tap leaks .. how to mend?

    hi , my kitchen sink tap drips and leaks at the shown joints in this image. How do i unscrew it to maybe replace the seals.

    have alook for a grub screw at the rear of the tap where the spout go's in.

    Ahh i see it .. There is a screw at the back .. can't see it clearly , what does it open with
