Laying Slate Tiles and Waterproof Membrane

I am laying slate tiles in the kitchen. I have just removed the old quarry tiles, which were broken and were originally laid on concrete. I have broken the old concrete away, and am ready to prepare the surface.

I intend to re-concrete the floor (about 20mm-30mm) before laying the slate tiles on top of floor tile adhesive.

1. Do I need to use floor tile adhesive or can I lay straight on to a second layer of concrete?
2. Do I have to create a "waterproof membrane"? If so what is it and how?
3. Is the slate tile (as a non-porous material) sufficient to prevent water penetration by itself, or would there be a potential problem of penetration through the grout joins?
4. Any other advice before I crack on?
