MDPE Stoptap Olive

Hello my MDPE stop-tap (25mm to 15mm copper) blew off when newly installed

Someone told me it's because I used plastic inserts when I should have used copper

I went to the merchant where I purchased the stop-tap asking for a copper insert

They said plastic inserts are always used

Is it just a case of tightening the stop-tap nut ?

Any help greeeaaatly aprreciated please

I've used both, in fact the push fit stop taps come with plastic inserts. I would put the olive back on & give it the grip of death.
