I’ve just had a wall opened up between 2 rooms and I’m going to need to sort the floor transition next week.
I opted to do it myself as I wasn’t initially sure what to do.
I’m planing to use a 12mm oak threshold, about 160mm wide.
I’m left with the original floor screed between the carpet and the LVT.
I was going cut out the DPC and then put down some self levelling compound to lay the oak on. Possibly screwing it down rather than using adhesive, but open to suggestions.

You'll need to make sure you've got a damp membrane of some sort under your new threshold of course.
Two part epoxy resin provides good resistance, but it's quite expensive.
Don't go putting big screws through your oak threshold into the concrete floor. The screws will rust and you'll get dark patches where they emerge through the timber.
Personally, I'd use a good grab adhesive.
You've got this!
