Only in the USA, thankfully.

Its my opinion that attitude and training of the firearms holder is paramount, certainly in all of the clubs I have visited to shoot at do not just pay lip service to safety but practice it.

This bit is key for me, and it is simple habits that have to be reinforced from day one. I think, like machinery, the dangerous time is that when the user becomes familair and pays less attention good practice eg I have been shooting with people who didn't 'break' their shotgun when climbing over a fence.
