Pathway mix?

I'm looking at extending path way by 2 flags wide to create a path to my front door so don't have to walk on mud. I need to raise it 6 inches so they are level so make a slab. So dimensions are 5m long 1m wide an 6 inch deep. Need to work out how many materials I'd need I. E cement sand n stone. An correct mix ratio If anybody has a idea. Have a go diyer here

At 6” you might want an edging in there first.
